We have created an Online Form for parents to fill out requesting a school transfer.
We have selected that Family 1 only have the option to update the form. When the Family 1 completes the online transfer request form, it sends an email to both guardians in Family 1.
Is there a way also have an email sent out to Family 2 (additional families) when Family 1 submits the transfer request online form?
When the custodial parents are not in the same Family, they would like the email go to both Family 1 and Family 2. It would also be…

Custom Forms Browse Student Name Key
We use the custom forms browse screen when creating processing lists of which guardians have/have not signed paperwork. As it is right now when you create a processing list it is an all or nothing list if sorting by signature because the field range is so broad. There is not a way to download a csv, eliminate the signed/unsigned field and upload to processing lists as Name Key / Other ID / State ID are not fields available on this browse. We would suggest adding one of these.

Ability to Select Banners on Custom Forms Requested
There are currently four pieces of information that will print on a student custom form. They are name, gender, grad yr/grade and other ID. Our district would like the ability to select the information we want on that banner. It would be helpful to remove other ID, which is just a local code, and add state ID and PIN which may be helpful to other districts receiving our forms.

Custom Forms - allow form
I think it would be very helpful to have an option that allows an Online Form posted in Family Access to automatically close after a parent submits it. Otherwise it is possible for parents to go back and edit what they have already done, and depending on the form this is very hard to try to manage. There are definitely times when you only want the parent to complete the form once. If the system can't automatically close the form once completed, perhaps an option can be built in to allow a processing list to be used to EXCLUDE students.…

In the online registration set up I would like to be able to add multiple Custom forms under 1 step.
I would like to be able to add multiple custom forms under one step of online registration. Perhaps Athletics and have 5 forms a,b,c,d,e instead of adding 5 steps to fill out 5 athletic custom forms. Same as the verify student information tab.

Custom Forms - Skyward Fields
Working on an Emergency Card/Form for our buildings to send home. We need "Custodial" and "Pick Up Rights" added to the Skyward fields so we can pull what we have on file and send home to parents.

Custom Forms - Add Skyward Fields
In the Student>Custom Forms, we would like to have some Skyward Fields added:
Advisor
Counselor
Discipline Officer
Teacher of Record
CY Homeroom
CY Homeroom Room Number
CY Homeroom TeacherWe would also like to have the option to choose how the student's name displays when we select the Student's Name field. Sometimes we will want to use the full name and sometimes we want just the first & last name.

Hide all tabs in Family/Student access until the parent has completed online registration.
We understand this is currently an option for the scheduling tab however we need this for ALL TABS. When a parent logs in they should see nothing but online registration. When a student logs in it would only show them a message that their parent needs to complete online registration before they can see anything. There are 6 other tabs a student can look at to figure the schedule/teacher. We have them disabled however it is a campus setting so we cannot enable them only to parents who have completed online reg. So for example Endorsements, this would be a…

Add Check Mark & Completed Date/Time for Step 2 of Online Form Process
Adding a check mark and the completed date and time to step 2 of the online form process would be instrumental in reducing parent confusion.

Custom Form/Advance Custom Form Watermark and Date/Time Stamp to show Current User
Update Custom Form/Advance Custom Form Watermark and Date/Time Stamp to show Current User filling out the form not the Family 1/ Guardian 1 as shown on the top when forms are printed. This can be used for legal verification of who filled in and submitted the form.

Each online form emailed to a different person
We set up a Back To School group of forms for parents to complete each year. Currently, we can only set up one person to get the email when someone completes all of the forms.
We would like to have an email go out per form. For example, the Health History form should go to the nurse, the Transportation Needs form should go to transportation, the Acceptable Internet Use form should go to the bookkeeper, etc.25 votes -
Ability to have mandatory online form at first login.
Getting parents to fill out emergency forms, computer acceptable use policies, etc. is burdensome. Other sites on the web make you click to accept their policies before entering the site. It would be a huge help to be able to require our parents to complete the forms prior to entering Skyward for the first time during the school year and for those forms to "pop up" on the screen so they wouldn't have to navigate through Skyward to find them.

HOME LANGUAGE SURVEY in Custom Forms
The Native Language and Home Language fields that are used for the EL Export to the state are not available in custom forms. We would like both fields available so we can create the HLS in custom forms, making it match the state so we are in compliance, and use it in the NSOE. We cannot do this at this time because the only field that is available is the Language field, which is a local field.

Add Homeroom Code and Homeroom Teacher to custom forms
Please consider adding Homeroom Code and Homeroom Teacher to the Skyward Fields available to pull onto the Custom Forms. A lot of times when we are setting up the Custom forms for parents to complete at the beginning of the year the forms that we want them to fill out require the homeroom field. At the high school and middle school level the students and parents don't always know who that is until the first day of school. We ask parents to complete these forms over summer. Ability to Clone CF from Entity to Entity
It would be more efficient for customers to be able to create a CF n one entity then clone it to other entities. Customers just starting to use CF are daunted by the task of having to create each form for each entity.
Any system fields in use would remain the same except for the entity designation. Any test/memo free-form fields would need to be created in the corresponding tables in the target entity.

Section 504 Custom Forms Report
Would it be possible to add a date range or school year parameter to the Section 504 Custom Forms Report so that when campus staff print the forms entered they can filter for current school year or at least a date range that falls within the school year?
Campus staff print the "ARD" packet for students annually to hand out to parents but the current functionality of the Section 504 Custom Forms report prints all custom form records regardless or school year.
