Health Records Vision and Hearing addition
This is our second year using your program. I noticed that when I use entry by home room for hearing and vision that what is required is not documented. I will begin by noting for you what needs documented then show you the nurse's proposal for modification of the entry by homeroom document.
All grades K-12 are required to have near and far vision screening and this documented. K+ or First grade are required to have Plus lens screening/Color vision screening and sterio depth perception screening completed and documented.
Attached is our proposal for entry by homeroom for vision screening.
Grades K+ through 3 and Emotional or Learning support students are required to be screened in hearing. Also 7th and 11th grades. The attached is our proposal for hearing screening documentation by Homeroom. The thresholds in Pennsylvania are as I documented 250/500/100/2000/4000 not 8000. And passing is 25 db but we document the db passing or not passing higher numbers in increments of 5.

When you Enter by Homeroom (WS\OF\HR\EH) or Enter by Class (WS\OF\HR\EC), try using the Add Health Screenings box. This was added later because of missing required reporting fields in the Add Vision Records and Add Hearing Records. There are more default options that may solve your issue.
There are no defaults/fields for near vision for us in Texas.For Hearing defaults you can add whatever thresholds you need, default 25db for each threshold, and as you add for each student can Edit the Threshold table to change the db or uncheck the passing box.
